Service hour certificate application

To apply for a proof of public service hours, please fill out a “record form” first and get a “proof of service hours” within the next 7 days since the activity is over. The maximum number of certificate applications we accept for each activity is: the total number of participants plus 5 (for instance, if there are 10 participants, we accept up to 15 applications).
